Important Information Before Booking:
 

• Intake visits require a $50 booking deposit, which will be applied to your total cost
• Booking Deposits are non-refundable
• Functional Medicine visits are self-pay only. Insurance cannot be billed for these visits
• All paperwork must be completed at least 24 hours before your appointment
• Please cancel at least 24 hours in advance to avoid a late fee
