We're so glad you're here. Booking online is the easiest way to schedule your next visit with Amber. Please read the information below before selecting your appointment — it will help make sure you're booking the right service and that your visit goes smoothly.
Before You Book
After You Book — Paperwork Required
After booking, you'll receive a confirmation email with a link to your patient portal. If a form doesn’t apply to you, do not fill it out — it will stay marked “Not Completed” on your portal, and that’s fine. We’re only looking for the forms that apply to you. For example: If you are not on Medicare, Tricare, or AHCCCS you do not need to complete those forms. If you are not signing up for a Membership, receiving Wellness Injections, etc., you do not need to complete those forms. Only applicable patient paperwork must be completed before your appointment. Patients who do not complete their forms in advance may have their appointment rescheduled.
If you have used OptiMantra with another provider before, please sign out of that account first. Each provider uses a separate portal. For login help, contact OptiMantra support directly — they handle all account issues.

Neurotoxin injections (Botox and Jeuveau), HA dermal filler (full Juvederm collection), SkinVive, and filler dissolution. All aesthetic treatments are performed by Amber Luna, FNP-C with a natural-first philosophy.
Botox and Jeuveau are priced at $13/unit. Tox Membership members lock in $12/unit. Filler pricing varies by area and syringe — Amber will confirm your plan at your visit.
New to filler? Your appointment includes a brief consultation before treatment.

Hormone therapy for men and women — including HRT, hormone pellets, and comprehensive wellness consultations. If this is your first visit at Vibrance, select Initial Consultation. Returning patients select Follow-Up.
Select peptide therapy is currently available at Vibrance for GH optimization, healing support, and metabolic health. Additional protocols are coming pending FDA approval. If this is your first peptide visit, select Initial Consultation. Amber will review your health history, discuss candidacy, and explain what is currently prescribable at your visit.
