Thank you for choosing Life Choice Health Center to provide you with healthcare services today. Please understand I will treat you with dignity and respect, and expect the same in return. I will fill all prescriptions on Wednesdays of every week to allow me the time to get your meds to you. Please request all refills within 7 days of your meds running out. I will send updates on all labs, diagnostic tests, and other vital information on Fridays, except for urgent matters, which will be done on the same day. 

Please understand that providing services to the community is vital, and a cancellation fee is applied. I do understand that things happen out of our control. This office will prescribe no ADD/ADHD/ or scheduled II drugs.
