LABS REQUIRED PRIOR TO INITIAL CONSULT:
Regardless of which program/service you are interested in labs will be required to assess your cellular wellness and metabolic health. You can chose to have them drawn in our office with a significant cash discount or we can email you a lab requisition and you can use the lab of your choice. Please chose an option and we will either send you a lab slip or ?reach out to schedule you for an in office lab draw.

Patients seeking appointments for lifestyle infusions/injections, Urgent care, or routine physicals can book online without an initial discovery call. FFS= Fee for service follow up visit, $125
Subscription= included with subscription, no charge

This option is for patients who being actively managed by another provider and will be following with them after their infusion.
We are not managing your ongoing care, we are managing your infusion. If you do not have a referring provider, please chose the ?INITIAL CONSULTATION option, as a full consultation and lab work is required.
We are pleased to offer Iron Sucrose Infusions for patients referred from outside providers.
In order to book, you will need to fax the following to our provider:
1) Recent labs- iron studies and CBC obtained within 30 days of infusion.
2) A valid infusion order from your provider- there is an order available on our website that you may download and ask your provider to fill out.
