Outside Referrals for Iron Infusions
We're pleased to offer iron sucrose infusions for patients referred by another provider who will continue managing their care. We manage your infusion only, not your ongoing care.
To book, please fax the following to 800-539-5792:

1. Recent iron studies and CBC (within 30 days of your infusion)
2. A valid infusion order from your provider 

No referring provider? Please book an Initial Consultation instead, since a full consultation and lab work are required.

Labs Before Your First Visit
Labs are required before your first appointment. You can have them drawn in our office or use your insurance at the lab of your choice. Select your option under New Patients Start Here, and we'll email your paperwork along with a lab slip, or reach out to schedule your in-office draw.

Coming soon: insurance-covered lab draws in our office through Quest Diagnostics, making it even easier to get your labs done on-site.
