If you have never been seen at Wellness Culture, you must schedule a New Patient Appointment, even if currently treated elsewhere.
Follow-up visits are for established patients only. 
 

Hormone and lab appointments require a phone consultation prior to any in-office visit.
 

A valid credit card is required to book. No-shows or cancellations under 24 hours incur a $50 fee.
 

Wellness Culture operates on a direct-pay model. Payment is due at the time of service.
