We accept Medicaid, Anthem BCBS, Aetna, Traditional Medicare, BCBS & Aetna Medicare, Devoted Medicare PPO. Please verify coverage before your visit.
Patient or responsible party pays all co-pays, co-insurance, and non-covered services at the time of the appointment. BCBS HMO Essentials and Exclusive Plans: Confirm in-network coverage for visits, you will likely need add Megan or Josh as your PCP for in-network coverage.

Self-Pay Rates (Credit, Debit, HSA accepted)
*Hormones, Weight, Sports, Mental Health: $200 initial / $135 follow-up
*Illness, Injury, Skin Care: $85–$135
*Physicals: $150 new / $125 established
*Lyons Residents Pediatric Sports Physical: $25
