For the fastest service, we encourage you to utilize the AI assistant, as she can answer most commonly asked questions immediately.
If you need to speak directly with Brittney, NP, Bridey Hayes, or Dr. Alonzo, you may request to leave a message with your name and number, and your provider will return your call as soon as possible. You may also request a transfer if your provider is available.
Upon arrival, please check in using the kiosk located in the lobby. If any annual paperwork or consent forms are needed, you will be notified prior to your appointment. Once checked in, please have a seat and your provider will call you back when ready.
A non-refundable $50 deposit is required for all booked appointments. This deposit will be applied toward your total at checkout unless you:
Vitamin injection appointments do not require a deposit; however, a card must remain on file. Our cancellation policy still applies. Missed injection appointments will be charged the cost of the scheduled injection.
All consultations are $50. If you choose to move forward with treatment the same day, the consultation fee may be applied toward your deposit.
