To ensure we are able to fully focus on you during the consultation please review the information below of necessary items to have completed ahead of time. Thank you for your cooperation.

Please have the requested forms filled out 48 hours prior to your visit to allow your provider ample opportunity to review them in advance of working together.
Please note, this practice is in-network with several commercial insurance plans, however if your plan is out-of-network, the self-pay fees are as follows: Sarah Early, Integrative Functional Health Consult - $300 per visit, Nutrition Consult - $175 per visit, and Craniosacral Therapy - $105 per session.
We require a credit card to be on file in order to guarantee all appointments. There are no charges applied to the credit card unless the appointment is missed or canceled with less than 48 hour's notice. There will be a $100 late cancellation and/or No Show Fee applied.
Information insurance is required prior to all consults.
Should you need to cancel or reschedule your appointment for any reason please notify Peony Women's Integrative Health 48 hours or more before the start time to make necessary changes.
