Please note that a credit card is required to confirm your booking. As part of our commitment to providing excellent service, we ask all clients to be aware of our no-show and late cancellation policies.

Late Cancellation & No-Show Policies: 


At Performance Hyperbaric, appointment times are reserved specifically for you. As a medical and wellness facility, we often have patients waiting to be seen, and last-minute cancellations directly impact our ability to provide timely care to others.
 

Credit Card on File & Late Cancellation and No-Show Fees: 


  • A valid credit card must be kept on file to secure all appointments. 
     
  • For Hyperbaric Oxygen Appointments, a $50.00 late cancellation and no-show fee will apply.* (See Below) 
     
  • For Physician and/or Medical Provider appointments, a $50.00 late cancellation and no-show fee will apply.* (See Below) 
     
  • For all other bookings, a $25.00 late cancellation and no-show fee will apply.* (See Below) 

     

Late Cancellation Policy & Fees: 


Appointments must be canceled or rescheduled within 12 hours of your scheduled time. 


Example: An appointment scheduled for 10:00 AM must be canceled or rescheduled no later than 10:00 PM the night before. Cancellations made after these times, or failure to arrive for the scheduled appointment, will result in a booking fee being retained. For Hyperbaric and Physician appointments, the booking fee is $50.00. For all other appointments, the booking fee is $25.00. Cancellations or reschedules made within 12 hours of the appointment, or failure to show up, will result in a  late cancellation fee charged to the credit card on file.


 

No-Show Policy & Fees: 


Patients who fail to arrive for their appointment without notice will have a No-Show Fee charged to the credit card on file.  For Hyperbaric and Physician appointments, the booking fee is $50.00. For all other appointments, the booking fee is $25.00. Repeated no-shows or late cancellations may require prepayment in full for future appointments or may result in restricted booking privileges.

 

Why this Policy Exists:


Our providers reserve time, staff, and specialized equipment for each appointment. When appointments are canceled late or missed, that time cannot always be offered to another patient who needs care. This policy allows us to remain fair to our team and to patients who are actively seeking appointments.

 

Exceptions:


We understand that true emergencies happen. Exceptions may be made at our sole discretion in cases of documented emergencies. Performance Hyperbaric understands that unforeseen circumstances and true emergencies may occasionally arise. While we strive to be reasonable and fair, all exceptions to this policy are made at the sole discretion of the clinic and are not guaranteed. The existence of an emergency does not automatically entitle a patient to a fee waiver.
 

Our providers, staff, and equipment are scheduled specifically for each appointment, and repeated late cancellations or no-shows disrupt our ability to deliver timely care to other patients. Accordingly, Performance Hyperbaric reserves the right to enforce this policy consistently and without exception in order to maintain clinical efficiency and access to care.
 

Patients who demonstrate a pattern of late cancellations or no-shows may be subject to additional booking restrictions, including but not limited to (a) Prepayment in full at the time of booking, (b) Scheduling by clinic management only, and/or (c) Limitation to designated appointment times, including off-peak or same-day availability only. These measures are not punitive but are implemented to ensure fairness, operational integrity, and continued access to care for all patients.



 

PATIENT ACKNOWLEDGMENT AND AGREEMENT: 


By scheduling an appointment with Performance Hyperbaric below, I acknowledge that I have read, understand, and agree to the Appointment Cancellation and No-Show Policy.


I understand that a valid credit card is required to secure my appointment and authorize Performance Hyperbaric to charge the credit card on file for applicable late cancellation or no-show fees in accordance with the policy.


I acknowledge that this policy applies to all appointments, including hyperbaric oxygen therapy sessions, physician or provider visits, and any other services offered by Performance Hyperbaric.


I understand that exceptions, if any, are granted at the sole discretion of Performance Hyperbaric and are not guaranteed. I further acknowledge that repeated late cancellations or no-shows may result in additional booking restrictions, including prepayment in full, scheduling by clinic management only, or limitation to designated appointment times.


By proceeding with scheduling, I confirm my acceptance of these terms and my responsibility to comply with the policy.
