Welcome to my private practice booking page. This is a fee for service practice. We DO NOT accept or bill insurance and we currently cannot provide services to patients who are Medicare/Medicaid recipients at this time due to federal regulations for cash-pay practices.
*All NEW patient appointments require a credit card on file and your card will be charged at the time of booking the appointment.*
Once you have booked your visit, you will get an email to setup your patient portal login and fill out all the necessary intake forms prior to your visit (**PLEASE CHECK YOUR SPAM FOLDER for the confirmation email.) All intake forms are required to be completed at least 24-48 hours prior to your appointment date to prevent any delays. Thank you, and I look forward to meeting you!
