Welcome to MP Integrative Medicine

Your appointment booking reserves your consultation time. During your initial evaluation, we’ll complete a comprehensive health history review, review available labs, and create your personalized wellness plan. No treatment decisions are made until your evaluation is complete.

Forms Required: Please complete all intake and consent forms in the patient portal prior to your visit. Appointments may remain unconfirmed until required forms are received.

Telehealth Details: Initial consultations are conducted via secure video. You must be physically located in Ohio at the time of your visit.

Lab Requirements: If you have recent lab work completed within the last 3–6 months, please have those results available. Additional labs may be ordered during your evaluation based on your health history and clinical needs.

Cancellation Policy: We require 48 hours’ notice to cancel or reschedule. Cancellations with less than 48 hours’ notice and no-shows may be charged a $50 fee.
