We're so glad you're taking this step toward better health. Please review the following before completing your booking:

Cancellation & Rescheduling Policy
We kindly ask for at least a 24 hours notice if you need to cancel or reschedule your appointment. Cancellations made with less notice, or missed appointments, may be subject to a $100 fee unless there was a medical emergency; certainly we understand life happens. This helps us keep appointment times available for all our patients.

Payment
3rd Day Health Revealed operates on a pay-as-you-go basis (no memberships). Payment is due at the time of service and a card is required to be kept on file when booking. 
Intake Forms
All intake questionnaires must be completed in the portal at least 24-48 hours prior to your visit so your provider can review your history and spend more time focused on you at your appointment than paperwork.
