Welcome to Wellhab! A few things to know before booking:

• We are a cash-pay practice — we do not bill insurance directly. 
• Florida residency is required for all services offered through this practice.
• Medical marijuana certification requires in-person new patient visit, per Florida law. This visit takes place at our Miami Beach location: 4302 Alton Rd Suite 420, Miami Beach, FL 33140. After your initial in-person exam, recertification visits can be completed via telehealth.
• HRT visits can be online via secure telehealth video — no in-person visit required.
• Cancellations require 24-hour notice to avoid a cancellation fee.
• All patients must have a valid, government-issued photo ID showing a Florida address (driver's license or state ID) as proof of Florida residency.
• After making your appointment, please check your e-mail to fill out your required forms at least 24 hours prior to your appointment.
