You will receive an email confirming your requested appointment time once your appointment has been approved.

A $50 booking fee is required for appointments, except clarity calls and injection-only visits. This fee will be credited toward your visit at checkout.

Please note: If you miss your appointment or reschedule less than 24 hours before your appointment time, the booking fee will be forfeited.

Many visits can be done by telehealth, including consultations and follow up visits. Just select the Telehealth Visit option when booking. You must be located in Illinois, Indiana, or Florida during your telehealth visit. Lab draws, IV hydration, injections, physicals, and DOT exams are in person only.
