Appointment Deposit Policy

We reserve time on our schedule just for you. Out of respect for our providers and other patients, please review and agree to the following.
Deposit & Consultation Fee
A $50 deposit is required to reserve your appointment. For eligible services, the $50 deposit will be applied toward any treatment scheduled within three months. If you choose not to move forward with treatment, the $50 deposit will be retained as the consultation fee.
Weight Loss & Hormone Replacement: The $50 fee is a consultation fee and is not applied toward or deducted from the cost of treatment, regardless of whether you choose to move forward with treatment.
Cancellation Terms
· We require 48 hours’ notice to cancel or reschedule an appointment.
· Rescheduling or cancelling within 48 hours of your appointment will result in the forfeit of deposit.
· No-shows will be charged a $50 fee or up to the full-service amount for longer appointments.
· Late arrivals of more than 30 minutes may need to be rescheduled and will be considered a late cancellation, resulting in forfeit of deposit.
· If you are sick, please notify us as soon as possible we are happy to reschedule as a courtesy if appropriate notice is given.
