All laboratory services are self-pay. Insurance is not accepted at this time.

If you do not have a physician’s order, provider order placement and review are included in your appointment.

A completed health questionnaire and signed consent are required prior to testing.

Abnormal results may require follow-up evaluation. Laboratory services are not a substitute for ongoing primary care.
