We do not accept insurance.
We are licensed to see patients in Florida.
We provide services for adults ages 18 and up.
Visits are by appointment only.
A $25 deposit is required to secure your appointment.
Deposits are not required for Direct Primary Care follow-up visits.
Your deposit will be deducted from the total cost of your visit.
Please complete your intake forms at least 24 hours before your visit
Please notify our office at least 24 hours before your appointment to reschedule or cancel without penalty.
