If you plan to use insurance, please return to our website and select the Insured Patient Intake link instead. You will complete a brief intake form and provide your insurance information for verification. If your insurance plan is accepted and your benefits are confirmed, you will receive a scheduling link within 1–2 business days.

• All appointments are conducted via secure telehealth.
• Payment is required at the time of booking.
• All appointment payments are non-refundable.
• HealingRoots Psychiatry does not prescribe controlled substances or stimulant medications. If you are seeking treatment that requires these medications, our practice may not be the best fit for your needs.
• If you are unsure which appointment type is appropriate, please complete the Prospective Patient Form on our website before booking.

Appointment Policies

Required Forms

All required intake forms must be completed at least 24 hours before your scheduled appointment. Failure to complete your forms by the deadline may result in your appointment being rescheduled.

Cancellation & Rescheduling

Appointments may be rescheduled one time with at least 24 hours' notice without forfeiting your payment.

Appointments canceled with less than 24 hours' notice are considered late cancellations, and payments are non-refundable.

No-Show Policy

Patients who fail to attend their scheduled appointment or arrive after the applicable grace period will be considered a no-show. No-show appointments are non-refundable and will require a new appointment and payment to reschedule.

Late Arrival

• Initial Evaluations: 15-minute grace period
• Follow-Up Appointments: 10-minute grace period

If you arrive within the grace period, your appointment will still end at its originally scheduled time. Appointment times cannot be extended due to late arrival. Arrivals beyond the applicable grace period will be treated as a no-show.
