To help us prepare for your visit and keep your appointment running on time, all required forms must be completed prior to your scheduled appointment. Please pay special attention to the ReNova Consent to Treat for form, as this form MUST be completed before we can provide treatment.
Appointment Scheduling Policy
A valid credit card is required to secure your appointment.
Your card will not be charged at the time of booking but will be securely kept on file for our cancellation/no-show policy. 
Appointment Cancellation Policy
Please provide at least 24 hours' notice if you need to cancel or reschedule.
By using this online booking portal, you agree to the no-show policy. 
There will be a $29.00 fee for missed appointments.
