-New patients should complete all intake forms prior to their appointment
-A valid photo ID is required for all visits
-Payment is due at time of service
-A $25 deposit is required to reserve your appointment and will be applied toward your visit
-Cancellations or resheduling requests must be made at least 24 hours in advance to avoid forfeiture of the deposit
-Hormone replacement therapy, weight management, and wellness services may require laboratory testing before treatment recommendations are made
-If you are scheduling a pre-operative evaluation, please bring (or email if telehealth appointment) all forms and any available laboratory or imaging results
