How it works:
1. Select the consultation that best fits your needs below.
2. After booking, you will receive an email to set up your secure patient portal and complete required intake forms.
3. Complete your consultation visit and receive individualized clinical recommendations.
Important booking info:
~ Consultation fee is required at time of booking.
~ If you enroll in ongoing care membership or 3-month care package within 30 days, your consultation fee is credited toward your first month.
~ 24-hour notice required for cancellations or rescheduling
~ No-shows or late cancellations may be charged the full visit fee
