Select a Service > Select a Date and Time > Complete Booking
After booking an appointment, you will receive an email containing new patient paperwork. This must be completed this prior to your first appointment or arrive 20 minutes early to complete it in the office.
If the time you want is not available or if you need assistance with scheduling an appointment, please call the office at (423) 597-1100.
Parking is available behind the building in our parking lot or across the street.
