We offer comprehensive non-DOT physicals for school, sports, and pre-employment requirements. Our goal is to provide a smooth, efficient experience while ensuring your safety and readiness for participation.
What to Expect:These visits include a general health evaluation and completion of required forms for school, sports, or employment. Please bring all necessary paperwork with you to your appointment. If forms are not provided, we are able to access standard Indiana school and athletic association forms when available.
Insurance & Payment:
  • If you are enrolled in Medicaid, services may be required to be billed through your insurance.
  • If you do not have Medicaid or Medicare, services may be paid out-of-pocket, or we can attempt to bill your private insurance; however, coverage is not guaranteed.
  • Payment is due at the time of service.
Important Health Information:For your safety, you must fully disclose any relevant medical history, including chronic conditions, past injuries, surgeries, or ongoing treatments. If you are currently under the care of a specialist—or have previously been restricted from work, school, or sports—you may need clearance from that provider prior to your visit.
If you have had a recent sports physical completed elsewhere, we reserve the right to review that documentation before proceeding with or completing your physical at our office.
Minors:All patients under 18 must be accompanied by a parent or legal guardian. Required health questionnaires and forms must be completed prior to the exam in order for the physical to be finalized and signed.

These visits are designed to improve access to care for individuals who may have difficulty attending in-person appointments, helping to reduce gaps in care and maintain ongoing connection with a healthcare provider.
What to Expect:Telehealth visits are focused on patient concerns and chronic condition management. They are structured to be time-efficient in order to accommodate all patients requesting services while maintaining provider scheduling and continuity of care.
These visits may be conducted via video and/or phone and may be used for sick visits, wellness visits, general health and wellness checkups, or management of routine and chronic conditions. Patients can expect their visit to last no more than 30 minutes.
Because telehealth services are cash-pay, they offer increased flexibility and accessibility; however, there are limitations. Certain conditions may require an in-person evaluation, physical examination, diagnostic testing, or referral to a higher level of care.
Scheduling, Payment & Billing:

Enrollment in the Weight Management Membership requires participation in a secure automatic payment system (ACH or EFT), with monthly charges billed to the payment method on file.

Membership includes a 10% discount on supplements, including but not limited to those purchased in-office or through the practice’s Full script dispensary. Discounts on select in-house wellness services are also included and may vary in availability and variety. Additional discounts on testing and services may apply depending on membership tier.
This program consists of functional and optimization-based services that are non-Medicare covered and cash-pay only. An Advanced Beneficiary Notice (ABN) is required prior to services.
Membership is $99/month and requires enrollment in autopay (ACH/EFT). Initial enrollment  fee is prorated and due at the time of enrollment. Membership may be canceled at any time with a required 30-day written notice via EMR Portal, prior to termination of billing.
