Appropriate for minor illnesses or concerns such as cough, sore throat, congestion, UTI symptoms, rash, nausea, pink eye, cold/flu symptoms, and other non-emergency needs. Patients with chest pain, severe shortness of breath, stroke symptoms, severe allergic reaction, or other emergencies should call 911 or seek emergency care.

Chronic care management visit for ongoing support, monitoring, and treatment of long-term health conditions such as hypertension, diabetes, thyroid concerns, asthma, high cholesterol, and other stable chronic issues. Includes medication review, care planning, refills when appropriate, and follow-up recommendations.

This visit is to establish care.  Please do not book this visit if you have already been seen by the provider.  This visit is only for new patients.
