 $25 booking fee applied-It will be credited to your visit but is non-refundable if canceled within 24 hours. 

Weight Loss Consult (30 min, In-Office)
Comprehensive NP evaluation to start or continue your program. We’ll review your history, goals, and labs from the past 6 months. If labs aren’t available, we can order them—labs are required before any medications are prescribed.
