Any appointment reservation is exclusively for you. If you do not cancel an appointment within 24-hours or NO SHOW your appointment, you will be charged $50 to the card on file.
After scheduling, please be sure to check your email for instructions regarding forms and patient portal links. These forms are required to be filled out at least 24 hours prior to your appointment or you will need to be rescheduled.
