**Please note:  A credit card must be placed on file at time of booking.  If you fail to cancel your appointment at least 24 hours prior to your appointment or you fail to complete your intake paperwork at least 8 hours prior to your appointment, your appointment will be cancelled and you will be charged a $100 convenience fee.  If you book your appointment less than 8 hours in advance, your paperwork still must be completed prior to your appointment, so please take this into consideration when booking your appointment to allow for adequate time to complete the paperwork.  We appreciate your adherence to our cancellation and paperwork policies, to ensure we can serve our clients as efficiently as possible.

We will obtain payment at the time of your service; we accept debit/credit cards.  Financing is also available through our payment partner, Cherry.

We still offer mobile IV services on a limited basis.  Please note that only two mobile visits are available per week; these are on a first-come, first-served basis.  You will be notified if there are no mobile services available for your desired week.  In addition, please note that a convenience fee will be added to all mobile services.  A $50 convenience fee will be added if you are within 30 miles of Oak Creek, and a $75 convenience fee will be added if you are outside 30 miles of Oak Creek.  We encourage you to book a clinic appointment if possible.  Thank you!
