These appointment times are for currently enrolled Direct Primary Care (DPC) members. One of the benefits of your DPC membership is having the time you need to discuss your health concerns without feeling rushed. Please choose the appointment length that best fits what you'd like to address during your visit. If you're unsure, it's always okay to choose a longer appointment.

15-Minute Appointment

• One simple concern or follow-up
• Medication refill
• Review of lab or imaging results
• Minor illness (such as a sore throat, sinus infection, or UTI)
• Blood pressure or medication check

30-Minute Appointment

• Two to three concerns
• New health issue requiring a more detailed evaluation
• Follow-up for chronic conditions (diabetes, high blood pressure, thyroid disease, etc.)
• Mental health follow-up
• Weight loss follow-up

45-Minute Appointment

• Multiple medical concerns
• Annual wellness visit with additional concerns
• Complex chronic disease management
• Women's health visit (Pap smear, birth control counseling, menopause discussion)
• More in-depth discussion of symptoms or treatment options

60-Minute Appointment

• Comprehensive annual wellness visit
• Multiple or complex medical concerns
• Extensive medication review
• Complex chronic disease management
• If you have several concerns you'd like to discuss during one visit or feel you may need extra time

Not sure which appointment length to choose? We recommend selecting the longer appointment. If we finish early, that's perfectly okay. Choosing the appropriate appointment length helps ensure we have enough time to address all of your concerns and provide the personalized, unrushed care that is a cornerstone of your DPC membership.

Welcome! This appointment is for patients who are ready to enroll in a Direct Primary Care (DPC) membership with Jessica Gerner, NP.

During your enrollment appointment, we'll review your medical history, discuss how the DPC membership works, answer any questions you have, and complete your membership paperwork.
One-Time Enrollment Fee (Due at Enrollment):

• Single Membership: $99
• Couple/Family Membership: $175

Please note that enrollment fees are one-time, non-refundable fees.

In addition to the enrollment fee, a monthly membership fee applies based on the patient's age and family status.
Monthly Membership Rates:

• Ages 20 and younger: $35/month
• Ages 21–64: $65/month
• Ages 65 and older: $85/month

Family and couple pricing is also available.

Your membership includes personalized primary care with longer appointments, direct access to your provider, same- or next-business-day appointments when available, telehealth visits, discounted laboratory and imaging services, and many in-office procedures at no additional charge.

Not ready to become a member? No problem! Non-member appointments are available for individuals who need care without enrolling in our Direct Primary Care (DPC) membership.

These visits are ideal for:

• Minor illnesses (cold, flu, sore throat, sinus infection, ear pain, UTI, etc.)
• Minor injuries
• Rashes or skin concerns
• Medication refills (when appropriate)
• Simple follow-up visits
• Sports physicals, DOT/CDL physicals, weight loss consultations, and other cash-pay services

Appointments are cash-pay only and payment is due at the time of service. Because this is a Direct Primary Care practice, insurance is not billed for office visits. If laboratory testing, imaging, medications, or referrals are needed, you may choose to pay cash or use your insurance at the outside facility when available.

Please note:

• Non-member appointments are intended for one primary concern. If you have multiple or complex medical concerns, additional time or a follow-up appointment may be recommended.
• Prescriptions, laboratory testing, imaging, or referrals are provided only when medically appropriate.
• Controlled substances and chronic pain management are not guaranteed and may require additional evaluation or referral.
• If you are experiencing chest pain, severe shortness of breath, signs of stroke, or another medical emergency, call 911 or go to the nearest emergency department.

• Initial Weight Loss Consultation: $75
• Follow-Up Visits: $60

Initial Consultation (30–45 minutes)
During your first visit, we'll discuss:

• Your medical history and current health
• Previous weight loss attempts
• Nutrition, exercise, and lifestyle habits
• Weight loss goals
• Whether prescription weight loss medications may be appropriate
• Development of a personalized treatment plan

Follow-Up VisitsThese appointments focus on:

• Monitoring your progress
• Reviewing weight changes and goals
• Medication management, if applicable
• Addressing side effects or concerns
• Adjusting your treatment plan as needed
• Ongoing encouragement and accountability

• Medication costs are not included in the visit fee.
• Laboratory testing may be recommended before or during treatment and is billed separately.
• Weight loss medications are prescribed only when medically appropriate after a thorough evaluation.
