Booking & Payment

Payment is required in full to reserve all new patient appointments.
Returning patients must keep a card on file for future visits.

After booking, you’ll receive two emails:
• Intake & consent forms
• Payment request invoice

Complete all forms promptly to hold your spot.
(Check your spam folder if you don’t see them!)

Cancellation Policy

Cancellations within 24 hours are charged 50%.
No-shows are charged in full.
This helps keep scheduling fair for everyone.
