Appointment Policy
When you book an appointment, that time is saved just for you.
If you cancel less than 24 hours before your visit or do not show up, a $50 fee may be charged to the card on file. This fee is for the time we saved for you and is not for medical care.
New patients MUST fill out all forms before their visit. If forms are not completed, the appointment may be cancelled or moved to another day.
By booking an appointment, you acknowledge your agreement to this policy and give us permission to charge the card on file if this policy is not followed.
