To provide timely, high-quality care for all patients, we ask that you review and respect the following appointment policies:
• Appointments must be cancelled or rescheduled at least 48 business hours in advance.
• Cancellations made with less than 48 business hours’ notice, no-shows, or arriving more than 15 minutes late may result in a late cancellation or no-show fee.
• Fees are charged to the card on file and are not billable to insurance.
• Patients arriving more than 15 minutes late may need to be rescheduled to ensure fairness to other patients.
• Repeated late arrivals may result in prepayment requirements for future appointments or limited scheduling availability.
• Two or more no-shows or late cancellations within a 12-month period may require:
– Prepayment in full to schedule future appointments
– Same-day appointment scheduling only
– Or dismissal from the practice at the provider’s discretion
• All intake forms, consents, and required questionnaires must be completed prior to your appointment time.
• Appointments cannot proceed if forms are incomplete.
• Patients who arrive without completed forms will be rescheduled and may be subject to a rescheduling fee.
• A valid card on file is required for all patients to cover applicable fees.
• By booking an appointment, you authorize Proficient Health & Wellness to charge applicable late cancellation, no-show, or rescheduling fees if policy requirements are not met.
