The co-pay (insurance) or total cost for the cash pay appointment will be automatically charged to your credit card on file on the day of your appointment. Scheduling an appointment is your confirmation that your credit card will be charged on the day of your appointment either the co-pay (insurance) or the total fee for the cash pay visit or for the late/no-show fee (if applicable) based on our late/no-show policy.
Canceling appointments must be done with a 48-hour notice to avoid the late cancelation fee. There is a non-refundable No-show and/or late cancelation fee of $50 that will be charged to an active credit card on file.Cancel your appointment within your patient portal.
To ensure we have all the necessary information for your care, we TENTATIVELY HOLD your appointment SLOT UNTIL YOUR SIGNED CONSENTS, INTAKE FORMS, QUESTIONNAIRES, AND ANY REQUIRED DOCUMENTS ARE FULLY UPLOADED. THIS MUST BE COMPLETED THE DAY YOU SCHEDULE AN APPOINTMENT. IF THESE ARE NOT COMPLETED WITHIN 3 DAYS OF YOUR SLOT, YOUR "APPOINTMENT" WILL BE CANCELED.
