Insurance: We accept Medicaid, Anthem Blue Cross Blue Shield Plans (EPO, PPO), Traditional Medicare and BCBS Medicare plans.
Please reach out to your insurance company prior to your appointment to verify ALL aspects of your coverage with respect to your appointment with us.
You are ultimately responsible for all deductibles, co-payments, and coinsurance amounts, as well as for any services provided by Lyons Health and Wellness that are not covered by your insurance provider. All co-payments and visit cost (if you have not met your deductible) are due at the time of your appointment.
If you have an HMO plan please verify with your insurance prior to your appointment that you are able to be seen by a provider other than your PCP and that your visit will be covered in-network.
Self Pay (Credit, Debit and HSA Cards Accepted):
Initial intake appointment – $165
Follow-up medication management appointment – $135
Payment Policy: We kindly request a credit card to be placed on file at the time of booking. Your card will not be charged until the time of your appointment. All co-payments and visit cost (if you have not met your deductible or are self-pay) are due at the time of your appointment..
Cancellation Policy: At Lyons Health and Wellness we do understand that life happens, if you need to reschedule you may do so online until 24 hours prior to your appointment time. Please call us at (303) 498-5941 if you have an emergency/extenuating circumstance and need to cancel the day of your appointment. There is no charge for same day cancellations with a phone call. No shows will be charged to the card on file for the full cost of the visit. Thank you for your attention to and understanding of this policy.
