Note: Please ensure that you provide a valid phone number and email address as this will be the primary mode of communication regarding your appointment. After booking your appointment you will receive an email which will allow you to sign a consent form and a brief intake form. It is important that we have your completed intake information prior to your appointment so that the provider can review your history and contact you with any questions/concerns.

We do require a valid credit card on file at the time of booking. Your card will not be charged for the service until after the service is provided.

Cancellation/No show fee: If you do not cancel or reschedule at least 24 hours prior to your appointment time, you will be charged a $50 cancellation fee using the card on file. By moving forward with online scheduling, you acknowledge and agree to this policy.
